RHAC PARENTS:
A reminder that Re-Enrolments for 2014 have now been completed and positions have been open up to external families. If you have overlooked re-enrolling your child for 2014 then you have until Friday 22nd November 2013 to get your paperwork in. Positions are filling quickly so get your forms in to avoid disappointment.
Your child’s position with KT OOSH @ RHAC in 2014 is not secured until the process listed below is fully completed. If we do not receive a re-enrolment form by close of business Friday 22nd November then we will assume you will not be requiring care in 2014 and your account will be closed effective 31st December 2013. If your account needs to be re-opened after Friday 22nd November then there will be an additional $25.00 administration charged applied to your account.
All re-enrolments received so far have been sent a confirmation email advising that there positions are secure. If you have not received one but have submitted your paperwork then please email us urgently to confirm rhac@ktoosh.com.au
If you DO require care for your child in 2014 you MUST –
1. Print and complete a 2014 Re-Enrolment form. We require one form per child. These are available on our website or click here 2014 Re Enrolment Form V1.2
2. Photocopy your child’s immunisation history record OR complete an exemption form. Exemption forms can be downloaded from our website AND must be signed by a Doctor. Click below if you require an exemption form, either Conscientious or Medical. Children with am exemption form will be excluded from the centre if there are any medical outbreaks:

3. If your child has asthma or allergies (including anaphylaxis) we require an updated management plan with an updated photo AND must be signed by a Doctor. To access this form click below for the form that applies:

4. You may drop your forms into the centre or scan and email to rhac@ktoosh.com.au
