Immunisation Records for KTOOSH

Effective from January 1st 2014 KT OOSH Services are required by law; Public Health Amendment (Vaccination of Children Attending Child Care Facilities) Act 2013 No 46 to keep a copy of every child’s immunisation record on file.

What this means for children attending KT OOSH Services is; One of the documents below  must be provided per child to the service upon Re-Enrolment or Enrolment for 2014 with KT OOSH Services. Failure to provide one of the three documents below and have this signed off by a medical practitioner (where applicable) will result in your enrolment not being accepted.

Provide one of the three documents below:

  • An ACIR** (Australian Childhood Immunisation Register) Immunisation History Statement which shows that their child is up to date with their scheduled immunisations, or;
  • An ACIR** Immunisation Exemption Conscientious Objection Form (IMMU12). This form is used if you wish not to immunise your child due to religious reasons, person choice etc.  This form must be certified by a medical practitioner and a parent/guardian, or;
  • An ACIR** Immunisation Exemption – Medical Contraindication Form (IMMU11) This form is used if you wish not to immunise your child due to allergic or medial reasons etc.  This form must be certified by a medical practitioner.

** All exemption forms need to be signed by a medical practitioner and provided by the parent to KT OOSH Services upon enrolment.

 A copy of your child’s immunisation details can be obtained at any time through the following ways:

  • by calling the Australian Childhood Immunisation Register on 1800 653 809
  • through Medicare Online Services at www.medicareaustralia.gov.au/online
  • by requesting a statement by emailing acir@medicareaustralia.gov.au
  • by visiting the local Department of Human Services Service Centre, Medicare office or Centrelink office
KT OOSH Services will advise in writing of any vaccine preventable diseases that come in contact with any aspect of the service.
