To Re-enrol your children with KT OOSH Services in 2014 you need to complete the following steps:

1. Click on this link 2014 Re Enrolment Form V1.1 to download and complete a 2014 Re-Enrolment form for each child. This form can also be found on our website www.ktoosh.com.au

2. Obtain a copy of your child’s Immunisation information OR completed exemption forms for unvaccinated children. * Information relating to immunisation records can be accessed in this newsletter by returning to the home page and clicking on the link called Immunisation Records Required.

3. Bring your completed 2014 Re-Enrolment form AND Immunisation history / exemption form and come into KT OOSH during one of the re-enrolment days noted below.

4. See enrolment staff to submit your Re-Enrolment form, immunisation documents,  check your previous enrolment information is correct & review the policy card.

We have nominated Re-Enrolments days when Guardians can re-enrol children with KTOOSH. These are the only days available when Re-enrolments will be accepted for 2014.

Rouse Hill Anglican College – Mon 9th & Tue 10th Sept 2013 from 3pm-6pm

Ironbark Ridge Primary School – Mon 16th & Tue 17th Sept 2013 from 3pm-6pm

Our Lady of the Angels – Fri 13th & Fri 20th Sept 2013 from 3pm-6pm

In addition to the above if you have a sibling starting kindergarten in 2014 and wish to enrol them with KT OOSH in the New Year you can download the 2014 form from the website, follow the steps and bring the form to this re-enrolment session.
