If you require care with KT OOSH @ IRPS in 2014 you should have submitted an 2014 Re-Enrolment form PLUS a copy of your child’s immunisation history statement by now. Confirmation of care in 2014 emails are currently being sent out.

If we are still waiting on further enrolment information from you IE Immunisation history statements or medical action plans you need to get these to your child’s service (or email through) ASAP so we can confirm your child’s position in 2014.

If you no longer require care in the New Year, please email irps@ktoosh.com.au to advise.

If you do require care in 2014 and have overlooked submitting your Re-Enrolment form, you need to go to www.ktoosh.com.au click on the KT OOSH FORMS / HOW TO ENROL tab and follow the instructions to enrol. Please note; You have missed the chance to complete a RE enrolment form so you need to complete the FULL 2014 Enrolment Form.

Your child’s position with KT OOSH @ IRPS in 2014 is not secure until 2014 Enrolment Forms and Immunisation History Statements have been submitted. This includes Medical Action Plans / Medication if required.
