A reminder that NEXT WEEK is your only chance to Re-Enrol your child for care with KT OOSH @ IBRPS in 2014!! 

Your child’s position with KT OOSH @ IBRPS in 2014 is not secured until the process listed below is fully completed.

From next term KT OOSH Services begin enrolling new families to the service for 2014. If current families have not submitted all paperwork required to re-enrol for care next year we will assume you do not require care and your child’s position with the service will be cancelled from January 1st 2014.

If you DO require care for your child in 2014 you MUST –

1. Print and complete a 2014 Re-Enrolment form. We require one form per child and forms can be downloaded from our website

2. Photocopy your child’s immunisation history record OR complete an exemption form. Exemption forms can be downloaded from our website AND must be signed by a Doctor.

3. If your child has asthma or allergies (including anaphylaxis) we require an updated management plan with an updated photo.

4. Bring forms and paperwork into KT OOSH @ IBRPS Next Week – Monday 16th Sept OR Tue 17th Sept from 3-6pm

When you submit your forms (we have arranged additional staff on site to accept these forms) you will be asked to –

• Read a policy card &
• Check over your other enrolment information that we have on file to make sure it is correct.

Please note if you have another child starting Kindergarten in 2014 you can bring their fully completed 2014 Enrolment form in and submit at the same time.

Morning care (6:45am- 8:45am) will be $16.00 per child.

Afternoon care (3:00pm-6:00pm) will be $18.00 per child.

Casual care (Morning or Afternoon) will be $21.00 per child.

Vacation care (7:00am-6:00pm) will be $48.00 per child.

Please note a newsletter will be emailed out later today (Wednesday 28th August 2013) that will contain the following important information –

1. How to re-enroll your child in 2014 with KT OOSH Services

2. Information relating to Immunisations

3. Additional changes to the service.

Effective from January 1st 2014 KT OOSH Services are required by law; Public Health Amendment (Vaccination of Children Attending Child Care Facilities) Act 2013 No 46 to keep a copy of every child’s immunisation record on file.

What this means for children attending KT OOSH Services is; One of the documents below  must be provided per child to the service upon Re-Enrolment or Enrolment for 2014 with KT OOSH Services. Failure to provide one of the three documents below and have this signed off by a medical practitioner (where applicable) will result in your enrolment not being accepted.

Provide one of the three documents below:

• An ACIR** (Australian Childhood Immunisation Register) Immunisation History Statement which shows that their child is up to date with their scheduled immunisations, or;
• An ACIR** Immunisation Exemption Conscientious Objection Form (IMMU12). This form is used if you wish not to immunise your child due to religious reasons, person choice etc.  This form must be certified by a medical practitioner and a parent/guardian, or;
• An ACIR** Immunisation Exemption – Medical Contraindication Form (IMMU11) This form is used if you wish not to immunise your child due to allergic or medial reasons etc.  This form must be certified by a medical practitioner.

** All exemption forms need to be signed by a medical practitioner and provided by the parent to KT OOSH Services upon enrolment.

Just a reminder of how to advise the service if your child is absent.

Please send a text message to the applicable number below to advise the service if your child is going to be absent from a previously confirmed session with KT OOSH. A staff member will always reply to confirm the message was received.

Please note this number is not monitored outside of OOSH Hours and a response from staff will only be sent around 6:45am or 2:45pm.

This is a short reminder for KT OOSH parents to please park out the front of the school when collecting and dropping off children to the service. Please do not (at anytime) park in the Tradesman’s entrance. This is a safety concern for both the school and KT OOSH and in the interest of the safety of the children please park out the front adhering to the street parking signs.
